I agree by my signature above, that I and my heirs, legal representatives, and assigns, hereby release, indemnify hold harmless, waiver all claims of any kind or nature and discharge the WCSDC, it's members, directors, coordinators, teachers, and other officers from liability to my person or property, including illness, injury, or death, incurred while participating in any club activity or other activity appearing in the club newsletter. Photos may be taken and used by WCSDC for purposes including but not limited to, promotional materials, newsletters, website, and Facebook, without compensation. I agree that this release, waiver, and indemnity agreement, is intended to be as broad and inclusive as permitted by laws of the state of Missouri and that if any portion therefore is held invalid, it is agreed that the balance shall continue to be in full effect. I Further understand that the information on this form is solely for the use of the WCSDC and that my personal information will not be distributed as part of a list for use outside the WCSDC
